Critically analyse and discuss the application of health psychology to the management of long term conditions with a focus on self-management.

At the end of the module the learner will be expected to be able to:

1. Critically analyse, discuss and debate government policies and priorities in relation to long term conditions

2. Critically appraise models of service delivery

3. Critically analyse and discuss the application of health psychology to the management of long term conditions with a focus on self-management

4. Explore and evaluate the evidence base relating to intervention programmes as applied to their own area of practice

Indicative Syllabus Content: Summary of Module Content

• Current government policies and priorities in relation to long term conditions

• Service Design and provision for patient centred care in long term conditions

• Health psychology and health behaviour concepts, models and theories

• Bio-psychosocial approach to designing, implementing and evaluating health and social care services

• Working in partnership with service users (e.g. Self-Management UK, shared decision making)

• Evidence-based best practice, applicable to individuals at different stages of the condition, from acute to community care.

• Introduction, setting the scene and outlining the specific ‘problem’ to be addressed by the service proposal

• Aims, objectives and clear description of the service proposal outlining key elements. You may wish to provide (as appropriate) here details of:

o Who will provide the service

o Who will have access to the service

o What exactly will the service provide

o How will the service be evaluated

• Justification for the service proposal based on evidence, best practice and health/social care or other relevant priorities

• Critical evaluation of underlying evidence and discussion of alternative models of service (if appropriate).
