What goals did the person have to achieve? Skills? Ensure you mention how they were at their own pace (344 PC 10)?

Establish skills for their own daily living.You have to give an example of each of the above and again be SPECIFIC not general .

You state how you did it and why and where, appropriate, linking into care standards, codes of practice and values(344 k6). give an example of when you praised the person (344 PC15).

This section should include a review of how the person was developing and managing their skills for daily living.

How did you recognize that this person had made progress? How did you assess this (344 PC20)? Who was involved in this process?

E. person, colleague, her manager, family. Did you highlight any areas for support? In what ways did you work with others to consider what the person’s future skills could be (344 PC21)?

Did you all work together as a team – how and why? Did you adapt to the changing needs of the person and the situation (344 PC22)?

eHow did you agree with others the way forward? What was the way forward and what was the plan to get you all going in the right direction (344 PC23)?

How and why was this person’s care plan updated? Was it updated in line with confidentiality, GDPR and your own company’s procedures (344 PC28)?

You could maybe think about linking in parts of codes of practice 1 or 2. It has to relate to your working practices with this person.

Now you will write your reflection. You should remember that you can use this to add to your CONTINUOS PROFESSIONAL LEARNING forms for the SSSC.

Reflect on how you think you dealt with this situation and remember that REFLECTION is crucial to your professional role and in terms of CPL for the SSSC. If you give an actual example of how this contributes to your own CPL you will cover 33 PC 19, 33 K5, 344 K7, 31 K7, 35 K7, 332 K7, 369 K7 .

It is also crucial that you examine your personal beliefs. Does your experience of older people and your own personal beliefs affect how you approach the work?
