Reflect on your clinical experience thus far and the EBP approaches you have learned in class, choose one thing you would like to see changed in your clinical site: a patient outcome, flow issue, communication issue etc.
Read through the Institute for Healthcare Improvement (IHI) instructions to create a Cause and Effect Analysis (AKA Fishbone diagram) which is posted on Blackboard. This also includes a fillable PDF for use to create the Fishbone diagram.
Write the effect you would like to influence in the box on the right side of the paper (10 pts)
In each of the 5 categories generate a list of causes and sub causes (see example posted on Blackboard) until each cause is specific enough to test a change and measure its effects. (60 pts)
Identify where you think a test of change should start and why (30 pts)
Exceeds expectations: 90-100
Meets expectations: 83-89
Partially meets expectations: 80-82
Does not meet expectations: ≤ 79
