Critically discuss, with reference to the relevant pathophysiology, the nursing assessment, priorities of care and nursing management of this acutely unwell patient for the first 24 hours following admission.

Assignment task
3000-word essay-based scenario assessing your management of an acutely unwell patient.

This assignment has been designed to provide you with an opportunity to demonstrate your achievement of the following module learning outcomes:

LO 1: Systematically explore and make relevant links to an in-depth knowledge base of anatomy, physiology, pathophysiology, and pharmacology that underpins the holistic care of adults in complex and unpredictable situations in a variety of healthcare settings.

LO 2: Demonstrate safety in drug calculations.

LO 3: Critically analyse, debate, and evaluate theories and concepts which underpin the delivery of safe, ethical, and professional practice.

LO 4: Demonstrate the ability to critically analyse and show confidence in the identification, observation, monitoring and holistic care of the adult with complex healthcare needs in a variety of healthcare settings using evidence-based practice.

LO 5: Critically reflect on their own performance in professional and interpersonal communication, teamwork and decision making in a wide range of complex healthcare situations.

Task requirements
This assignment will focus around the theme of the module. The aim of this assessment will allow the student to demonstrate an understanding and safe management of the acutely unwell patient to prevent deterioration and the promotion of recovery. The pass mark is 40%. The purpose of this assignment is to assess the students’ knowledge and critical understanding of anatomy, physiology and pathophysiology applied to the acutely ill adult. Their ability to identify appropriate means of observing and monitoring patients and their ability to critically reflect on the communication skills required for autonomous practice will also be examined and meet learning outcomes 1,3,4 & 5. Your seminar, skills and ALERT Leaders will help you build your knowledge around this subject throughout the module. During the final week of the module there will be a lecture to help you plan your revision.

SCENARIO
You are to choose this scenario for your 3000-word (+/- 10%) essay-based scenario:
Question 1:
Fazia is a 52-year-old woman who has been taking regular Ibuprofen tablets for her rheumatoid arthritis.
Fazia has been experiencing abdominal discomfort for the past 24 hours but has continued taking her over the counter ibuprofen to ease the pain. This morning Fazia experienced three episodes of vomiting fresh blood.
Fazia is brought into the Medical Assessment Unit by Ambulance with a provisional diagnosis of a gastro intestinal bleeding.
You go to see Fazia, and she appears pale and looks drained and lethargic. She complains of epigastric pain and feeling dizzy. When you check her vital signs, you find the following parameters:

Respiratory rate: 24 breaths per minute
Oxygen Saturations: 90% on room air
Pulse: 120 beats per minute
Blood pressure: 85/53 mmHg
Capillary refill time: 3 seconds
Temperature: 35.4 degrees centigrade

Instructions:

Critically discuss, with reference to the relevant pathophysiology, the nursing assessment, priorities of care and nursing management of this acutely unwell patient for the first 24 hours following admission.

Your answer should also demonstrate your underpinning knowledge of relevant pharmacology, psychosocial aspects as well as any potential complications.

1.5-line spacing and double spacing between paragraphs. Font and size: Arial, 12
The assignment should be present in an academic essay format, with a clear introduction, body and conclusion. It should demonstrate and appropriate formal academic style, avoiding contractions (e.g. don’t, won’t, etc.) informal or spoken forms of language.

Headings and subheadings should not be included.
Your work should be written in the third person.

Introduction:
This should succinctly outline in sequence, the main points you will discuss in your
assignment.
This should be linked to the instructions of assigned work and the relevant
learning outcomes of the module.
The relevant practices in relation to the NMC code should underpin your practice.
The narrative will be written in the 3rd person.

Main body:
Define the underlying condition and the relevant pathophysiology/physiology (GI bleed)

Define shock and briefly outline the types of shock with more emphasis the type
shock to be explored. (Hypovolaemic shock, cardiogenic shock, and septic shock) (Patient is having a hypovolaemic shock).

Pathophysiology of shock to be discussed. Define stages of shock.

Physiological changes of the body cause by hypovolaemic shock: cardiac output, Heart rate, Blood pressure, system vascular resistant SVR, Jagular venous pulse JVP, Ejection fracture, skin temperature.

A systematic approach to assessment should be demonstrated and in context of
the presented case scenario/study. This should also include: (A-E assessment)
(NEWS and SBAR)

The evidence-based life-threatening interventions/management

Critically explore the relevant implications – pathophysiological and for
practice

The relevant tests/investigations/interventions to manage the underlying
condition. (look at British thoracic society, for oxygen)
Relevant pharmacology (IV paracetamol, analgesic LADDER)
The ongoing nursing care management including the holistic/psycho-social to
promote recovery.
Evidence of collaboration and inter-professional working
Recognition of improvement or deterioration and the appropriate actions.

Critically discuss, with reference to the relevant pathophysiology, the nursing assessment, priorities of care and nursing management of this acutely unwell patient for the first 24 hours following admission.
