Population Health Management
Throughout the semester you surveyed a sample population and put together a Community Health Needs Assessment (CHNA) following the PYA blog and the 2019 San Francisco CHNA. For the final project submit a PowerPoint Presentation that includes:
Give a general overview of Assignments 1-4. (no more than 6 slides).
What did you find to be the most difficult portion of conducting a community health needs assessment?
Group work and teammate location. The most difficult part of this assignment was working as a group while living in different time zones and different states with varying life/work schedules. Also conducting a survey without the entire group being present.
What is your impression of hospital community benefit and do you ultimately believe that hospitals should be responsible for conducting community health needs assessments?
How can hospitals use needs assessments, asset maps, and other public health tools to plan community health interventions?
What is your impression of how hospitals have implemented population health management techniques to date?
