Health Promotion Assignment
Use the nursing process to develop a health promotion plan of care for a community client in one of the following age groups: infant (1 month to one year); toddler (13 years); preschooler (36 years); school age (612 years); older adult (>60 years). Selecting an adolescent age group (1318 years) is also permitted with prior faculty approval.

Assignment Description
For this assignment you are asked to identify an infant/child or an older adult who will serve the role of a client. Using a variety of course resources, you will conduct a complete health history, a general survey, and optionally a vital signs measurement on this client. Your meeting with the client can be virtual (i.e., Zoom) or inperson; phone calls are not appropriate.

After completing the assessment, you will use the findings to create a developmentally appropriate, evidencebased health promotion care plan. You will also reflect on the therapeutic communication techniques used during the interview, as well as address one motivational or adherence theory to promote successful achievement of the outcomes.
Anticipated time commitment (preparation and execution) for this assignment is approximately 810 hrs.

Steps for Completion:
Use the Health Promotion Template for your documentation. Note that you will need to expand each box in the Word doc to fit the contents of your write up. Remember not to leave any segment blank (blank means that information was not collected e.g., client does not recall past birth history). You also need to submit the completed assignment in Word doc into Canvas by the designated deadline.

Section A: Obtain a verbal consent from your client prior to your scheduled meeting date by disclosing the following: a) description of the project, b) what to expect on the day of the visit, including time commitment and the nature of the assessment, c) the limitations of your scope and role, and d) an assurance of anonymity. You must always provide your client’s initials in the writeup to maintain your client’s anonymity. Document your responses in each respective box.

Section B: Using clinical terminology, provide a detailed description of the health history assessment findings. Important: You must show evidence of deep understanding of the course content. Use all resources at your disposal to build your answers (i.e., Ch. 1, 2, 4, 7, 18, 19, and 21 of your textbook, relevant prerecorded lectures and other instructional activities posted on Canvas). You can use the
textbooks of other nursing courses (e.g., Nursing Diagnosis Handbook).

Section C: Anthropometric measures can be obtained subjectively. Use a BMI calculator specific to pediatric or geriatric clients.
Complete a comprehensive general inspection that addresses 1) appearance, 2) body structure and position, 3) body movement, and 4) emotional status, disposition, and behavior. Use clinical terminology to document your findings. Vital signs measurement is optional. Use at least two screening tools in your assessment; see below for some suggested options.

Section D: Provide a detailed description of 1) the expected development for your client’s age group, as well as 2) the assessment findings specific to your client. Based on Erikson’s or Piaget’s Theory, state your client’s developmental stage with the expected  development for that stage. Compare your client to these milestones and address any noted discrepancies/delays. Ground your responses in the relevant textbook chapters, Canvas module resources, and selected outside sources.
Identify one motivation or adherence theory and describe how you would apply it to secure full cooperation from the client and meet the care plan goals. Examples include:
Maslow’s Hierarchy of Needs, Theory of Selfefficacy, Adherence & Concordance, and Selfdetermination Theory of Motivation. See prerecorded lecture for additional information.
Describe the specific therapeutic communication techniques used during the interview, along with an evidencebased rationale.
Describe the ways in which you altered the interview based on the age and developmental stage, functional and neurosensory status of the client.

Section E: After analyzing the assessment findings from the previous sections, identify one health promotion problem that your client has or is atrisk for developing. Complete a brief research on the selected topic and write up your findings to include overview of the problem, its significance (provide data to support), and risk factors. Identify the prevention level most appropriate for your client and list the preventive strategies to address the problem.
