Do a writing that;
Educate patient on the importance of medication compliance in order to prevent further attack.
Place patient on and a combination of ICS (Inhaled corticosteroid) and LABA (Long acting beta agonist) for acute asthma exacerbation post discharge or a combination of ICS and LAMA (Long-Acting Muscarinic Antagonist).
Educate on ways to identify asthma triggers
Encourage patient to start using rescue inhalers at the start of symptoms.
Refer the patient to a care Manager/Social worker /Financial counselor to help with issues involving insurance/Access to medications
Follow up at the chest clinic for further testing on lung functions
