Employment is seeking out a passionate and self-motivated individual to work as a full-time bi-lingual social worker for the Aging In Place Outreach Network (APON), the new non-traditional referral network into its highly respected continuum of care for older adults in Upper Manhattan. The Outreach Coordinator will require both strong clinical assessments skills as well as excellent community outreach abilities.
RESPONSIBILITIES include, but are not limited to the following:
Conducting outreach and establishing connections within the community
Developing training materials for local networks of community members with no background in geriatric care on identifying signs that an older adult may need support age in place safely
Referring older adults in need of support to the appropriate Y aging in place services
Conducting home visits for initial assessments and care planning
Providing supportive counseling
Maintaining electronic records
Tracking data and trends through an online relational database
Building relationships with community partners
