Create two diagrams by identifying and distinguishing the differences in SDOH (Social Detriments of Health) for each of the clinics with proficiency achieved with the identification of at least 4 SDOH identified per clinic (3 SDOHs need to be related to factors outside of the clinics and 1 SDOH needs to related to factors internal to the clinic (provider expertise, training, type of physicians).
