Description of scenario 
Evelyn is 87 years old, which puts her at higher risk for certain health problems due to the natural ageing process. She has hypertension (high blood pressure) and rheumatoid arthritis, both of which are chronic medical conditions that can affect her physical health and well-being. She has recently been diagnosed with a urinary tract infection (UTI), which is a common problem in older adults and can cause symptoms such as urinary frequency, dysuria (painful urination), and urge incontinence. Evelyn has been distressed by the death of her husband, James, which occurred five years ago. She is anxious about maintaining a healthy diet to prevent illness and possibly prevent dying from the same cause as her husband. She is feeling unwell and may be experiencing distress or anxiety related to her symptoms and hospitalisation. Evelyn has become dishevelled and is repeatedly looking for her compact powder, which could indicate a decline in her cognitive function or memory. She is repeatedly asking if she is dying, which could be a sign of anxiety or distress related to her health and the loss of her husband.
Evelyn lives on her own at home, which could make it more difficult for her to manage her medical conditions and maintain her physical health. Evelyn’s daughter Matilda lives 35 miles away and visits weekly but is not able to provide constant support and care for Evelyn. Evelyn is no longer able to do gardening, which was a previously enjoyed activity, which could affect her mental and emotional well-being. She needs help with washing her back and legs, which could indicate a need for additional support with activities of daily living.
