Description of needs assessment with the justification of the need for new agency and demonstration that there will be no duplication of services that address the social problem that your team has identified. Description of agency, including the mission statement, vision statement, and services.
Organizational chart. An organizational chart that includes brief job descriptions, including qualifications for each position (voluntary or paid) and salary details.
