Discussion Post: My first clinical experience was at a Lincoln hospital on the med surgery unit. This experience made me re-examine and analyze a lot about my path and reaching my goals in achieving a career in nursing. I felt that begin able to get a feel and touch of the working environment is extremely important to nursing student.
I was very nervous but at the same time I was so curious while sitting down and waiting in the main entrance lobby for my clinical professor, only to know what the clinical setting would be like and providing care to all different type of individuals from all paths of life . Shortly after my clinical professor made her way down to main lobby and introduces herself and took us on the unit that we will working on for the day. She started the day by having a morning conference with us, where we get to know a little about each other and what was expected of us on our shift. Then we got a tour on the floor, where we were been introduces to other staff and clients. She pointed out safety precautions we need to take into account and proper PPE needed to be worn before entering and leaving a client room , we also look at the different storage rooms or locker rooms in cased we need to get an equipment’s or other items are needed for a producer we know exactly where to go and get it and where and how to properly dispose items when we finish uses them.
Then it was time for me to meet and provide care for my clients for day. Before I did that, my clinical professor let me gather all the necessary information that I need to know about my client so I can provide the best possible care for her without causing any harm. I enter the client room, I perform hand hygiene , then I introduced myself to my client by stating my first and last name, I also let my client know why I am here and if it’s ok for me carry out this producer today, I ask her to verify her first and last name and following her date of birth even tough I knew my client was unable to access.
I was assign to do head to toe access on my client with the supervision of my clinical introducer by starting out with general status such as checking vital signs ,Heart rate, blood pressure, temperature, pulse oximetry, respiratory rate and pain was unable to access and I also listen to lungs sounds and bowel movements in all four quadrants. Both me and my team member was assign to give our client a complete bed bath. First we gather all the necessary supplies we need such as towels, linen, wipes, socks , gown etc, we provide client privacy as needed, client’s activity tolerance for bathing was asses, hand hygiene perform , we both worn gown and gloves, Client’s skin was asses for abnormalities, height of bed was adjust to comfortable working level. Siderail was lower on working side, eye care was done I wipe each eye from the inner corner outward. Client’s upper extremities was wipe down by uncover the client’s arm closest to me, underarm, arm, and hand was wipe and dry. Siderail was raise client was been move to the opposite side of the bed and we repeat the steps above with the other arm, for chest and abdomen, client chest was cover with a bath blanket. Blanket was lower to the client’s umbilicus, exposing the chest and abdomen. chest and abdomen were wipe down. When wiping the lower extremities I expose the leg closest to me . I wipe client’s leg and dry, moving from the ankle to the knee and then knee to groin, then wipe foot, especially between the toes and thoroughly dry foot. Place siderail up and move then to the other side of the bed. Steps above was repeat with the other lower extremity. When it was time to wash the client’s back, we turn the client to a side-lying position, with her back turn to me. Wipe and dry the client’s back. I also gave my client a backrub because it teaches the patient how to relax and promote good circulation. For perianal care my teammate did it which she wipe front to back and also she secure the foley catheter form kinking and become obstructed. Linen replacement was done, we apply clean top sheets, blankets and pillows.
My last assign for day was obtaining a Capillary Blood Sample for Glucose Testing from a different client. Upon arriving the client room, I perform hand hygiene and put on gloves, I Introduces myself, let the client aware of why I am here, got his permission to go ahead and check his blood glucose level. I confirm the client first and last name following his date of birth. Then I turn on the gluomomitor and scan the MRI code to ensure I was with the right client. I ask the client which finger would he like to prick, I clean his preferred finger with alcohol swab, then massage it for a bit for blood flow. Then I place a test stripe in the glucomonitor, then prick the client finger the first drop of blood I clean it off. Then took the second drop of blood and place it on the test stripe. While waiting for the result I clean the clients finger and then communicate finding with client him.
