Case Study: Assessment & Intervention

THE INTENDED OUTCOME OF THIS ASSIGNMENT IS FOR STUDENTS TO:

1. Demonstrate foundational assessment and intervention skills.

2. Illustrate the use of assessment tools to collect, organize and interpret information.

3. Formulate an intervention plan based on assessment information.

DIRECTIONS:

Using an example from your current or previous social service work experience, complete an assessment using an assessment tool and include:

a. A brief summary of the person(s) to be used in this assignment.

This should be in paragraph format. Please modify specific details pertaining to the individual(s)/family and use non identifying information to ensure anonymity and confidentiality are maintained. (Depending on the complexity and number of persons in the case this section could be up to 1 page in length).

b. A brief summary of the agency that provides the context for the assessment (1 page).

 What services are provided by the agency?

 What is the mandate of the agency?

 How did the individual come to the attention of the agency?

c. An assessment with accompanying summary of assessment findings (approximately 4 pages plus assessment tool).

 Identify and describe the social work theories used for the assessment.

 Consider what you would want to know more about? Why is that area of interest?

 Identify key areas for intervention using references to relevant literature.

 Consider what area would provide the greatest return on the investment of effort into an intervention? Or what area would be most likely for there to be success? Or what area must be intervened in due to risk to self or others?

 This section MUST include a: genogram and ecomap; and any other tool you are familiar with to strengthen your overall assessment process.

d. An intervention plan (2 pages minimum).

 Ensure the interventions are related to your assessment.

 Goals must be written in SMART format and include objectives (steps for achieving the goals).

e. A reflection summary (2 page minimum)

 A description of the individual’s perspective in addition to any distinct insights or recommendations from your own perspective.

 A description of theories relevant to this example which helped to foster an enhanced understanding of this individual/family situation.

 Reflect on the approaches and roles you have undertaken as a social worker in this situation.

 Identify how existing social policy impacted the assessment and intervention.

 Consider and outlines the challenges, rewards and constraints experienced in this situatiion
